Our Agency is a community based program that provides intermittent skilled professional services and related clinical care to the patients who are in transition from the inpatient experience to home environment. All care is provided under the supervision of the patient’s physician.

After initiations of the services our clinicians make arrangements to provide patients with all the necessary medical equipment and supplies. Training on the use, maintenance and cleaning of the equipment is provided as necessary.
