Main Activities
- Ensure that care is provided according to all relevant policies, procedures and regulations and approved schedule
- Ensure that Home Care Workers (HCW) are properly trained to administer at home care, monitor and evaluate all assigned tasks and resolve any conflicts that arise.
- Schedule work assignments with regard to HCW training and experience
- Coordinate Case Management- Take referrals as offered and assign cases with cooperation with RN.
- Provide all required documentation for Payroll/Billing as scheduled.
- High school diploma required; college degree preferred.
